When To Use
Use G715™ when an ACORD Certificate of Insurance does not fully capture the specific coverage details, additional insured requirements, or endorsements required on the project. This document is suitable for any size project.
What It Does
G715 supplements an ACORD Certificate of Insurance by capturing the specific coverage details and endorsements the standard ACORD form does not fully accommodate. It gives owners a more complete insurance verification document, reducing gaps in coverage confirmation that can create liability exposure during construction. The Supplemental Attachment form should be completed, signed by the contractor’s insurance representative, and attached to the ACORD certificate.
Additional Information
Some states regulate insurance verification documents. Check applicable state laws before using or requiring this form. G715 was approved for use in New York on October 5, 2018.
